Breast Augmentation
Breast Augmentation or breast implant surgery is a procedure that enhances the volume and shape of the breast. If you've felt for some time that your breasts are too small, then breast augmentation surgery might be a good option for you. According the American Society of Plastic Surgeons, in 2007, over 347,000 women underwent a breast augmentation procedure, making breast augmentation the most commonly performed cosmetic surgery procedure in the United States. Patients typically fall into one of three categories: women that have lost volume in their breasts as a result of aging, woman with unequally sized breasts, and women that simply want larger breasts.
Types of Breast Implants
The two types of breast implants available on the market today are saline filled implants and cohesive silicone gel filled implants. We commonly refer to these implants by their shorter name, 'saline implants' or 'silicone implants'. The newest cohesive silicone implants were approved for cosmetic use by the FDA in 2006, after extensive research into their safety and performance. The principal advantage to the silicone implants is that the breast has a more natural 'feel', and there is less visible implant wrinkling. The principle disadvantage to silicone implants is cost as silicone implants cost about $400 more per implant than saline implants. We use both Allergan and Mentor breast implants for cosmetic breast procedures.
A third option exists for patients interested in a very natural appearing breast augmentation. Your own fat can be removed during a liposuction procedure and transferred into your breasts. A Lipo Augmentation procedure can achieve about a one cup size increase in breast volume. Larger augmentation volumes can be achieved when the BRAVA expansion device is used for a few weeks prior to Lipo Augmentation. The advantage of the Lipo Augmentation procedure is that a very natural appearing breast augmentation is combined with a body liposuction procedure for a complete breast and body rejuvenation.
Incisions for Breast Implants
Both saline implants and silicone implants can be placed through three types of breast augmentation incision locations:
- Infra-mammary (under the fold of the breast)
- Peri-areolar (near the nipple area)
- Axillary (underarm area)
Although most smaller silicone implants (less than 275-300 cc) that can be placed through an axillary or peri-areolar incision, larger silicone implants require an infra-mammary incision. Both saline and silicone implants can either be placed under or over the chest muscles (pectoralis muscle) depending on your anatomy and the 'look' that you are interested in achieving.

Most patients have the implant placed beneath the pectoralis muscles, and the implants appear to last longer and perform better in this submuscular position.

What 'look' are you interested in?
During your consultation, you will be asked what type of 'look' you wish to achieve with your breast augmentation. To make this discussion easier, we have devised a series of four 'looks' that describe the appearance of breast augmentation results.
- Stage 1: The result appears totally natural and proportional. The causal observer could not tell if the clothed patient has had a breast augmentation.
- Stage 2: The result appears proportional to the patient's height and weight, but is a generous improvement to the bust line.
- Stage 3: The result is an 'augmented' look with rounded fullness in the upper pole and exaggerated cleavage. It does not appear to be natural.
- Stage 4: The result is an obviously augmented 'look', using an implant that changes the shape of the breast significantly, which looks 'fake'.
How do I find a breast implant that is the right size for me?
The process of determining the correct size of breast implants for you is based on a three-part process. The first part considers your answer to the augmentation 'look' question about Stages 1 through 4 (see above). The second part of the implant sizing process is precise measurements made of your chest dimensions. The implant must fit your chest width and your breast tissues in order to look good and last for a long time. The third part of the process involves trying on some 'sizer' implants in front of a mirror while wearing a bra, and deciding which size is too large, and which is too small. Combining the decisions made from this three-part process allows us to help you decide on an implant size that is right for you.
What type of anesthesia is used for breast augmentation surgery?
Breast augmentation surgery is done under IV sedation anesthesia. This technique relies on intra-venous sedation that makes you very comfortable and unaware of any activity around you. While asleep, you are breathing on your own while you are constantly monitored by an anesthesia provider. The main advantages of twilight anesthesia are: (i) it does not require putting a breathing tube in the throat, (ii) it does not require a breathing machine, (iii) the recovery is much faster, (iv) there is much less "hang-over" from anesthesia, (v) and there is much less nausea. All these elements translate into greater comfort and safety. Breast augmentation surgery takes between one and two hours to perform, and is done on an outpatient basis. The procedure can also be done under a general anesthetic if you wish.
What is the recovery process after breast augmentation surgery?
After a breast augmentation procedure, you will be comfortable enough to go out to dinner on the evening of your surgery. Light narcotic pain medication and Lyrica are prescribed. Most breast augmentation patients are back to full-time non-strenuous work within one week. For two weeks after the surgery, you will wear a special bra that keeps the implants positioned properly and aids in the healing process. You may resume light, non-impact exercise (walking, stretching) immediately after surgery. Jogging and cycling can be resumed 6 weeks after surgery. Weight lifting can be resumed 8 weeks after surgery, but push-up and bench-press type of weight lifting is not recommended.
More about breast augmentation incisions
There are three commonly performed incisions for breast augmentation surgery. These are, below the breast (infra-mammary), around the 'nipple' (peri-areolar) and through the armpit (axillary). Each of these incisions has advantages and disadvantages. There is a fourth incision, made in the umbilucus, which is not performed in this surgical practice.
The infra-mammary incision is placed within the fold of the breast, and is generally well concealed beneath a bra or bikini top. It provides direct access to the pectoralis muscle and is the most common approach for placement of silicone breast implants. Within about a year, the surgical scar softens and lightens in color, making it difficult to see, even when unclothed. The length of the scar varies based on the size of the breast implant. Generally, very short scars (2-3 cm) are used to place saline implants. Longer scars (4-5 cm) are used to place silicone implants. The difference in incision length is because saline implants are filled after they are placed within the breast. Silicone implants come pre-filled and thus require a larger incision for placement.
The peri-areolar incision is placed at the junction between the darker areola skin and the lighter breast skin, along the lower border of the areola. This scar tends to heal very well. The size of the areola can limit the volume of silicone implants that can be placed. This incision will permit the use of a moderately sized silicone implant in most cases. The scar is visible only when a patient is unclothed. The areolar incision can also be incorporated into a breast lift procedure, if necessary.
The axillary incision is placed within the looser skin of the armpit. It also heals very well. Since the incision is rather remote from the ultimate position of the implant, a video-endoscopic technique is used to accurately develop the pocket beneath the pectoralis muscle that will accept the breast implant. Larger silicone implants are difficult to place through this incision. In this surgical practice, the axillary incision represents a small percentage of the augmentations that are performed.
What about Fat Transfer Breast Augmentation?
Your own fat from a gentle liposuction procedure can be transferred to the breast to achieve a very natural breast augmentation without the use of a breast implant. There are several terms used to describe the technique: lipo augmentation, fat graft breast augmentation and fat transfer breast augmentation. Our clinic has considerable experience with fat transfer for facial enhancement. The equipment and techniques are now available in our clinic for Lipo Augmentation of the breast. Ideal candidates for this procedure are patients with youthful breast skin and no breast or nipple laxity. For most patients, a maximum of one cup size of augmentation can be achieved with fat grafting. Use of the BRAVA external breast tissue expansion device can be used to enhance the results of Lipo Augmentation. The amount of fat required for Lipo Augmentation can be substantial. For this reason, good candidates for Lipo Augmentation should also have a sufficient amount of body fat. The fat is removed using a special liposuction technique that minimizes trauma to the fat. Regions of the body that are good donor sites for graftable fat include the abdomen, love handles, inner and outer thighs and the arms. The Lipo Augmentation procedure takes significantly longer to perform in the operating room compared to a breast augmentation using implants. Touch up surgery is sometimes required in order to balance the appearance between the two breasts. Fat Transfer to the breast can also be used to correct breast assymetry and to soften the appearance of existing breast implants.
